Approval Application Form

Please print out and complete the following two application forms:

• Page 1
• Page 2

Please also print and complete the following form for each staff member:

• Staff Qualifications Form (Page 1)
• Staff Qualifications Form (Page 2)

Please complete and return with your application:

• the enclosed pages for all relevant staff, outlining their qualifications as per the sports categories.
  
  
• a copy of the relevant sections of your operations manual.
  
  
• a copy of your relevant equipment inventory.
  
  
• proof of public liability insurance.

If copies of qualification awards are required, you will be notified in due course.

Please note that the monitor visiting your centre will require sight of:

1. staff rosters.
   
   
2. activity programmes.
   
   
3. details of client numbers for a specific week/weeks.
   
   
4. safety statement
   
   
5. signatures testifying to staff having read the Safety Statement and Operations Manual.
